Qualifications:

·            Rebate recipients must live in Davis, Weber, Morgan or Summit counties. 

·            The products purchased must be installed on properties within those counties.

·            Original receipt must be attached unless sent in by fax or e-mail.

·            Only the products on the approved product list qualify for rebate.
